Our Story
KindlyMoments was born in 2021 inside a hospital discharge planning office. Our founders — a cardiologist, a nurse practitioner, and a health technology engineer — watched the same heartbreaking pattern repeat itself: patients left the hospital hopeful, but without adequate support, many returned within 30 days sicker than before.
Nearly 1 in 5 Medicare patients are readmitted within 30 days of discharge. Each readmission costs the healthcare system an average of $15,000 — and costs patients and families immeasurable stress, pain, and lost confidence.
We asked a simple question: What if we could keep the care team connected to the patient after they leave the hospital? What if families had real-time visibility into their loved one's recovery? What if early warning signs could trigger intervention before a crisis?
