Why We're Building This
Nearly 1 in 5 Medicare patients are readmitted to the hospital within 30 days of discharge. Each readmission costs the healthcare system an average of $15,000 — and costs patients and families immeasurable stress, pain, and lost confidence.
We saw a gap that technology could fill. After discharge, patients are often left navigating complex medication schedules, monitoring vital signs, and recognizing warning signs — all on their own. Families feel helpless. Care teams lose visibility. And preventable readmissions keep happening.
KindlyMoments is being built to change that. We're creating a HIPAA-compliant care coordination platform that keeps patients, families, and healthcare providers connected during the critical post-discharge recovery window. Real-time monitoring, intelligent alerts, and a family-friendly dashboard — all designed to catch problems early and prevent unnecessary hospital returns.
We're currently in our beta program with select healthcare providers, with our full public launch planned for Q2 2026. We believe that with the right technology and a compassionate approach, we can make a real difference in patient outcomes.
